Short Term Acute Care Hospital (Northeast)

OBJECTION: Client wanted a solution to reflect the needs of chest pain/MI/NSTEMI and heart failure patient population. In addition, they wanted to promote their great cardiac program.

SOLUTION: Cardiac Care Discharge Folder
A folder was designed with highly-engaging tips for a heart healthy lifestyle. The folder’s content included tips to maintain a healthy heart, how to read blood pressure test results and how to access the online patient portal for additional resources. The artwork also showcased their department’s accreditation and details surrounding their cardiac program.

Senior-Population Solution

WHO: Marketing Manager // Emergency Department
Short Term Acute Care Hospital (Northeast)

OBJECTION: An Emergency Department wanted to improve their discharge process to accommodate the hospital’s demographics which were primarily senior citizens. It was desired to have a multi-purpose resource which assisted in the discharge but also could be utilized at home in the event of an emergency.

SOLUTION: Mini, Magnetized ER Folder
A small pocket-style folder, backed by a magnet was constructed. It was designed with emergency phone numbers listed boldly on the outside with plenty of room to store folded paperwork, securely inside. A magnet was added to the back so patients could place it visibly at their home (such as on a refrigerator), seen by family, caregivers and EMTs in the unfortunate event of an emergency.

RESULT:

  • Consolidation of cost associated with patient discharge documents
  • Resource for Emergency Personnel, family and caregivers to reference

ER & Car Safety Initiative

WHO: Director of Clinical Informatics // Emergency Department
Critical Access Hospital (Southwest)

OBJECTION: An Emergency Department experienced a considerably sizable number of car-related incidents. The hospital wanted to play an active role in educating their community.

SOLUTION: ER & Car Accident Folder
A folder which not only assisted in the discharge of ER patients was also designed with emphasis on safe practices when riding in vehicles. The folder’s artwork included driving safety guidelines, car seat safety tips and seat belt reminders, in addition to vital information pertinent for improved care transitions.

RESULT:
